The Hospital Elder Life Program Geriatric Team

HELP serves as a catalyst to stimulate the development of interdisciplinary clinical geriatric expertise. As a result, the hospital earns peer recognition as a center of excellence in the provision of geriatric care.

Geriatric professionals form the backbone of the Hospital Elder Life Program. Members of the HELP Team include:

Elder Life Nurse Specialist (0.5 FTE)*

  • A masters prepared nurse with experience in geriatrics
  • Provide comprehensive geriatric assessment and interventions in collaboration with nursing staff and other disciplines
  • Provide educational programs for nursing staff
  • Coordinate interdisciplinary rounds

The Elder Life Specialist (1.0 FTE)*

  • Unique role created for HELP
  • Bachelors degree (masters preferred) in human services or healthcare related field, with experience in geriatrics, and supervisory experience
  • Excellent communication and organizational skills
  • Responsible for day-to-day operations of HELP and coordination of all volunteer activities
Geriatrician (0.1 FTE)*
  • Participate in HELP interdisciplinary rounds
  • Provide clinical consultation
  • Provide educational programs for physicians and nurses

Program Director (0.1 FTE)*

  • The role of the Program Director can be assumed by the Geriatrician or the Elder Life Nurse Specialist
  • The program director oversees and supervises the entire program, and is key to provide overall leadership

Interdisciplinary Support Staff (as needed)

  • Participate in HELP interdisciplinary rounds
  • Consultations available upon referral by HELP staff should include:
    • chaplain
    • pharmacist
    • dietitian
    • rehabilitation therapists
    • discharge planner
    • social worker
    • psychiatric liaison nurse

Volunteer Services

  • Screen potential candidates to identify suitable HELP volunteers
  • Provide standard hospital orientation and oversight

* Note: FTE's are to serve 200-250 patients per year.
